Abstract

Pressure Injury Education and Staging Decision Tree to Mitigate Pressure Injuries. Dr. Grace Dengler, DNP, MSN, BS-CS, BSN-RN

Objectives:

  • Decision-making in pressure ulcer prevention ensures the right actions are taken at the right time—tailored to the patient’s condition, supported by evidence, and coordinated across the care team. Without it, care becomes reactive rather than proactive, increasing the risk of preventable harm.
  • Clinical staff members will demonstrate an increase in pressure injury knowledge by 20% upon completing the pressure injury education
  • Registered Nurse (RN) staff will complete the Braden scale assessment and documentation upon a patient’s admission or readmission 80% of the time after completing the pressure injury education
  • Nurses including Registered Nurse (RN) and Licensed Practical Nurses (LPN) will complete 80% of the time skin integumentary assessment and documentation using the EMR Avatar upon admission/readmission and during weekly skin checks
  • Clinical staff members will implement appropriate pressure injury measures based on PI staging decision tree 50% of the time
